2021 FLAG Gala Registration Terms and Conditions
As an attendee of the 2021 FLAG Gala, offered by or on behalf of FLAG (the “Organization”), you hereby acknowledge your awareness and knowledge of the risks and consequences associated with the transmission of, exposure to and potential infection and sickness from the COVID-19 virus resulting from any such in-person attendance at the Organization’s facilities. Further, you hereby represent that you understand and acknowledge that the COVID-19 virus is extremely contagious and is capable of being spread from person-to-person with or without the use of protective equipment, sanitizing measure and social distancing practices.
By attending this event, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk of exposure to COVID-19 by attending any in-person event and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death. You further acknowledge and understand the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of you or others including, but not limited to, FLAG’s employees, volunteers, and program participants and other attendees. You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury, illness, damage, loss, claim, liability, or expense, of any kind, that you may experience or incur in connection with you attendance at the Organization or participation in Organization events (the “Claims”).
I HEREBY RELEASE, COVENANT NOT TO SUE, DISCHARGE, AND HOLD HARMLESS FLAG, ITS EMPLOYEES, AGENTS, DIRECTORS, OFFICERS, MEMBERS AND REPRESENTATIVES, OF AND FROM THE CLAIMS, INCLUDING ALL LIABILITIES, CLAIMS, ACTIONS, DAMAGES, COSTS OR EXPENSES OF ANY KIND ARISING OUT OF OR RELATING THERETO. I UNDERSTAND AND AGREE THAT THIS WAIVER AND RELEASE INCLUDES ANY CLAIMS BASED ON THE ACTIONS, OMISSIONS, OR NEGLIGENCE OF THE FLAG, ITS EMPLOYEES, AGENTS, DIRECTORS, OFFICERS, MEMBERS AND REPRESENTATIVES, WHETHER A COVID-19 INFECTION OCCURS BEFORE, DURING, OR AFTER PARTICIPATION IN THE 2021 FLAG GALA.
